Find a Conveniently Located Care Provider From the State Fund MPN

Welcome to the Provider Finder, your link to the medical providers in the State Fund MPN. If you experienced a work-related injury or illness, the Provider Finder can connect you with doctors and specialists in our medical provider network (MPN) who will provide prompt, professional treatment.

Use the Provider Finder to search a directory of State Fund MPN providers and facilities. For your convenience, you may customize your search with several specifications, including provider distance from your home or work place, provider name, urgent-care facilities, and regional listings.

If you're unable to locate a provider in the State Fund MPN, please contact the Medical Access Assistants or your claims adjuster to access other providers in Anthem’s Blue Cross Network.

Medical Access Assistants (MAA)

Should you need assistance locating an available MPN physician or scheduling a medical appointment, and are not able to reach your claims adjuster, medical access assistants are available to help you with scheduling and confirming your physician appointments during doctors’ normal business hours. They are available to assist you, in English and Spanish, from 7:00 AM to 8:00 PM (Pacific Time), Monday through Saturday (except holidays) by phone at (888) 782-8338 or by fax at (800) 371-5905. You may also email your appointment request to StateFundMPNMAA@scif.com.
